Welcome to the Online Telecare Assessment.

You will be completing this form on behalf of the person that you have assessed for Telecare.

Please be aware that this form is designed to be completed within one sitting. You are not able to save the form to complete later. If you are not active for over 25 minutes, the form will expire. 

Before you start

Please have the person’s LAS (Liquid Logic) ID number to hand. If you cannot access LAS, please call the Telecare team 01452 583 784 | 01452 583 774 who will happily advise you what this is.

What happens once I complete the TOP form?

When you have completed the online form, you will receive an automated email with a copy of the assessment you have completed. Please keep this for your own records.

The Telecare Team will also receive a copy of this email and from this they will process the relevant documentation to arrange the installation of the Telecare equipment that you have requested.
